Most diets restrict a food group you can picture: no meat, no gluten, no dairy. A low iodine diet restricts a single trace mineral that hides inside foods with no obvious connection to the ocean or a salt shaker. Milk. Egg yolks. A slice of commercially baked bread. A red-tinted vitamin capsule. None of these announce themselves as iodine sources, and that is exactly why this diet trips people up.

The low iodine diet, usually shortened to LID, is not a lifestyle choice. It is a short, medically directed protocol followed almost exclusively by thyroid cancer patients in the one to two weeks before radioactive iodine (RAI) treatment or a diagnostic RAI uptake scan. The goal is narrow and specific: deplete the body's stable iodine so that whatever thyroid tissue remains, cancerous or not, absorbs as much of the therapeutic radioactive iodine as possible. Get it right, and treatment works better. Get it wrong by eating a cheese sandwich on iodized-salt bread, and the whole point of the diet can be undermined without a single symptom to warn you.

This guide covers where iodine actually hides in the food supply, what the evidence says about how strict and how long the diet really needs to be, and how that plays out against a food-labeling system that was never built with iodine restriction in mind.

Why Thyroid Cancer Patients Restrict Iodine Before Treatment

Radioactive iodine therapy works because thyroid tissue, and most differentiated thyroid cancer, retains the thyroid's natural habit of pulling iodine out of the bloodstream to build hormone. A dose of iodine-131 gets taken up by any remaining thyroid cells along with ordinary dietary iodine, delivering a targeted radiation dose to exactly the cells doctors want to destroy. If a patient's body is already saturated with iodine from food, the radioactive dose has to compete with all of that stable iodine for the same uptake pathway, and less of it reaches the target tissue.

The American Thyroid Association instructs patients preparing for RAI remnant ablation or therapy to follow a low iodine diet, and the 2015 ATA Management Guidelines set the working threshold most clinics still use: less than 50 micrograms of iodine per day, typically for one to two weeks beforehand. That is a steep cut. The FDA's daily value for iodine is 150 micrograms, and the ThyCa Low-Iodine Cookbook notes that a single teaspoon of iodized salt can contain more than 400 micrograms on its own, well above the daily target for the entire diet.

There is no single global standard, though. The British Thyroid Association also recommends one to two weeks without a strict microgram ceiling, while the European Thyroid Cancer Taskforce recommends three full weeks. A cohort study on LID utility found that even four to five days of restriction meaningfully lowers urinary iodine excretion, and Dutch and South Korean studies both found four days to a week statistically indistinguishable from a full two weeks. In practice, the exact number of days a patient is told to follow is closer to institutional habit than settled science.

It is also worth being clear about who this diet is not for. Cleveland Clinic endocrinologist Ravali Veeramachaneni puts it bluntly for patients with ordinary hyperthyroidism or Graves' disease outside of a scheduled RAI treatment: "We don't recommend a low-iodine diet for most people with hyperthyroidism. It's just not practical and can keep you from getting the nutrition you need to keep the rest of your body healthy." Iodine is so embedded in the food supply that restricting it long-term, rather than for a defined two-week window before a specific procedure, creates more nutritional risk than benefit for most people.

The amount of iodine packed into ordinary foods varies wildly, and a handful of everyday staples carry far more of it than most people would guess.

A low iodine diet has nothing to do with sodium. Plain kosher salt, canning salt, and any salt explicitly labeled non-iodized can be used freely. The problem is that most table salt sold in the United States has been fortified with potassium iodide or cuprous iodide since 1924, and under federal rule the package must say so: iodized salt is legally required to carry the statement "this salt supplies iodide, a necessary nutrient." That labeling requirement is the single most useful thing an LID patient can look for at the grocery store, because it turns an invisible ingredient into a printed sentence. Sea salt is restricted too, on the assumption that anything harvested from the ocean carries meaningful ambient iodine.

The catch is processed food. Packaged and restaurant food rarely discloses which type of salt was used, and the ThyCa cookbook notes that manufacturers are not obligated to specify iodized versus non-iodized salt on an ingredient list, nor can they always guarantee which their suppliers used. That uncertainty is exactly why the ATA tells patients to avoid restaurant meals altogether during the diet window: there is no reliable way to know which kitchen reached for the iodized shaker.

Dairy, Seafood, and the Additives Nobody Expects

Milk itself does not naturally carry much iodine, but modern dairy farming does. Iodine-based sanitizers, called iodophors, are routinely used to disinfect cow teats before and after milking and to clean milking equipment, and a controlled trial in the Journal of Dairy Science found that spraying an iodine-based post-milking dip measurably raised milk iodine concentrations, with levels ranging from roughly 164 to 252 micrograms per kilogram depending on the dipping method. Combine that with iodine-fortified cattle feed and dairy becomes, per the Healio historical retrospective on US salt iodization, one of the largest and most unpredictable sources of dietary iodine in industrialized countries today, milk, cheese, yogurt, butter, and milk chocolate included. Anyone who has already learned to spot hidden dairy names on a label for a milk allergy has a head start here, though the LID cares about the presence of dairy itself rather than casein or whey specifically.

Seafood and Seaweed, in Wildly Different Amounts

Fish, shellfish, sushi, and anything harvested from the sea are on every LID avoid list, and for good reason: ocean life concentrates iodine directly from seawater. But the concentration varies enormously by species. A GC-ECD analysis of commercial seaweed products found kombu (kelp) at extraordinarily high levels, commonly measured in the thousands of milligrams per kilogram, while nori, the seaweed used to wrap sushi rolls, measured far lower. Both are still restricted on the LID, because even nori's comparatively modest iodine content adds up quickly against a 50 microgram daily ceiling.

Whole eggs and egg yolks are restricted; egg whites are not, and are one of the diet's few reliable protein staples. Soy is restricted for a different reason entirely: the ATA notes that soy itself is not a meaningful iodine source, but high soy intake has been shown in animal studies to interfere with how efficiently the thyroid takes up radioactive iodine, so soy sauce, soy milk, and tofu are cut regardless.

Then there is the additive list that reads like a chemistry exam. Potassium iodate and calcium iodate are used by some large commercial bakeries as dough conditioners, which is why "commercially prepared bakery products" carry their own specific warning on the ATA's brochure. Carrageenan, agar-agar, and alginate, all derived from seaweed, are grouped with kelp and nori as precautionary avoid items even though no controlled study in our research turned up a precise quantification of how much iodine these processed extracts actually retain; it appears to be a caution based on origin rather than a measured risk. Blackstrap molasses is specifically flagged as high in iodine, while regular unsulfured molasses is fine. Any vitamin, supplement, or herbal product containing iodine, kelp, or dulse is off-limits, and patients are told to check with a doctor before stopping any red-colored or iodine-containing medication.

The strangest item on the list is a food dye. FD&C Red #3, also called erythrosine, is a genuine organoiodine compound: its molecular structure contains four iodine atoms bonded directly to a xanthene ring. It shows up in maraschino cherries and occasionally in pink or red beverages, cereals, candies, and vitamins, which is why ATA and ThyCa both call it out by name. In a twist that will slowly retire this item from LID lists, the FDA revoked authorization for Red #3 in food and ingestible drugs on January 15, 2025, citing carcinogenicity data under the Delaney Clause, with manufacturers required to reformulate by January 2027.

A Global Paradox: The World Is Trying to Add Iodine, Not Remove It

Here is the tension worth sitting with. Everything a low iodine diet asks a patient to avoid is, from a public health standpoint, exactly what most of the world has spent a century trying to put back into the food supply. Iodine deficiency causes goiter and, in its most severe form during pregnancy, permanent cognitive impairment. WHO and UNICEF have promoted universal salt iodization since the 1990s as one of the most cost-effective public health interventions ever deployed, and global household coverage of iodized salt has climbed from under 20 percent in the early 1990s to roughly 86 percent today.

Regulatory approaches diverge sharply by country. In the United States, iodization has been voluntary since 1924, and today only about half of table salt sold nationally is actually iodized, with most processed food using non-iodized salt. Europe is a genuine patchwork, and the FDA's own regulatory comparison04941-3/fulltext) notes that Australia and New Zealand mandate iodine fortification of salt while the US does not.

India sits at the opposite end. Universal salt iodization has been government policy since the early 1960s, and a 2011 food safety regulation restricts the sale of non-iodized common salt for direct human consumption, as confirmed in a 2018 government press release to the Lok Sabha. That mandate became genuinely contested in 2025, when a regional High Court bench quashed the restriction on procedural grounds, prompting public health experts to warn in outlets like the Deccan Herald that undoing decades of progress over a technicality risks reversing hard-won deficiency reductions.

None of this changes what an LID patient needs to do for two weeks. But it explains why the diet feels so unnatural: it asks you to swim against a food system, and a global health infrastructure, that has spent a century engineering iodine into the ordinary foods you already eat.

Does the Diet Even Need to Be This Strict?

The honest answer is that the evidence behind LID is thinner than its rigid reputation suggests. A mixed-methods systematic review and meta-analysis found only very low certainty evidence that a patient's iodine status meaningfully affects ablation success, and the ATA's own 2025 guideline update acknowledges that supporting data is largely retrospective, drawn from historical controls rather than randomized trials. The same review found no clear evidence that a two-week diet outperforms a one-week diet, or that a stricter diet outperforms a moderately restricted one.

That review also documented what patients themselves report as the hardest part: not the biochemistry, but the psychological weight of the restriction, confusion over ambiguous food lists, and real difficulty for anyone whose baseline diet leans heavily on iodine-rich staples, seaweed and soy-based cooking in South Korea and Japan being the clearest example. Patients frequently pre-freeze LID-safe meals in advance, since post-treatment fatigue makes cooking from scratch difficult right when the diet needs to continue for a few days after the RAI dose itself.

A Practical Label-Reading Strategy

Avoid entirely:

  • Iodized salt and sea salt, and any packaged food that lists either as an ingredient
  • Fish, shellfish, sushi, and all seaweed, including kelp, kombu, nori, wakame, and dulse
  • Carrageenan, agar-agar, and alginate (also listed as algin), all seaweed-derived additives
  • Dairy products: milk, cheese, yogurt, butter, ice cream, and milk chocolate
  • Egg yolks and any food made with whole eggs
  • Soy sauce, soy milk, tofu, and other soy-based products
  • Commercially prepared bakery products made with potassium iodate or calcium iodate dough conditioners
  • FD&C Red #3 (erythrosine), found in maraschino cherries and some pink or red-tinted beverages, cereals, candies, and vitamins
  • Blackstrap molasses (regular unsulfured molasses is fine)
  • Any vitamin, supplement, or herbal product containing iodine, kelp, or dulse
  • Restaurant and fast-food meals of any kind, because the salt type used cannot be verified

Limit, do not eliminate:

  • Grain products such as bread, pasta, and pastries, generally capped around one slice of bread or a half cup of pasta daily
  • Beef, chicken, and turkey, in modest portions
  • Homemade bread made without soy, dairy, or iodized salt is generally fine in normal amounts

Generally fine on the diet:

  • Non-iodized salt (kosher or canning salt), used freely
  • Egg whites
  • Fresh or frozen fruits and vegetables
  • Unsalted nuts, peanut butter, and unsalted popcorn
  • Plain rice, oats, and unsalted crackers

Regulatory context that affects what a label can tell you: In the United States, iodized salt must, by federal rule, state that it "supplies iodide, a necessary nutrient," but disclosure of iodine content on the Nutrition Facts panel is voluntary unless iodine was deliberately added as an ingredient, in which case declaring it becomes mandatory. That means a product can legally omit iodine from its nutrition panel even when its ingredients (dairy, iodized salt, iodate dough conditioners) make it high in iodine. Outside the US, expect no consistency at all: Australia and New Zealand mandate iodized salt, much of the European Union treats iodization as voluntary, and India restricts the retail sale of non-iodized salt for direct consumption, though that rule has faced recent legal challenges.

A shelf-by-shelf checklist:

  1. Check the salt statement first. If a product's label says "supplies iodide," or simply lists "salt" with no non-iodized qualifier, treat it as iodized.
  2. Scan the ingredient list for dairy in any form, including whey, casein, and milk solids in products that do not read as obviously dairy.
  3. Look for "egg" versus "egg white" specifically. Whole egg and egg yolk are restricted; egg white is not.
  4. Search for carrageenan, agar-agar, or alginate in the additives section, especially in dairy alternatives, desserts, and processed meats.
  5. For baked goods, check for potassium iodate or calcium iodate among the conditioners, or default to avoiding commercial bakery items you cannot verify.
  6. Check red or pink-tinted foods and vitamins for FD&C Red #3 or erythrosine by name.
  7. Treat all restaurant food as unverifiable and skip it for the diet's duration.
  8. Set aside all vitamins and supplements unless you have confirmed they contain no iodine or kelp.
